Why Update the Consult Form?

  • The Consult Form in Therap can serve as a written prescription when updated with the right language
  • This helps reduce delays in starting new medications and simplifies communication between the pharmacy and provider
  • The same update should be made to the Medication Review Note in Therap

📝 Note: This only applies to programs using the Consult Form for medication changes and reviews. If you’re not sure, check with your agency’s Therap administrator.

What to Add to the Consult Form

Update the form’s default wording to include the following statement:

“Unless otherwise noted, start this medication with the next compliance package cycle. I authorize a 30 day supply with 11 refills (5 refills for controlled substances) unless otherwise indicated.”
  • This language allows Neighborhood LTC Pharmacy to process medication changes directly from the written form
  • It clearly outlines dispensing instructions and refill limits

⚠️ Caution: Be sure staff understand this message will be used as a legal prescription unless additional notes are added.
