Notify Neighborhood LTC Pharmacy Early
- Provide as much notice as possible when an individual is being discharged
- Include the anticipated discharge date and any important details
- Email the Neighborhood LTC Pharmacy location that supports your program
💡 Tip: Advance notice allows the pharmacy to prepare medications so they can go with the individual.
Sending Medications at Discharge
- Neighborhood LTC Pharmacy can fill and send as many medications as possible for the individual’s transition
- Share any special packaging needs or information about who will be managing the medications after discharge
- Include details about the new caregiver, facility, or family if known
- Medications can be sent to the new facility if needed
⚠️ Caution: If notice is not provided ahead of time, there may be a delay in sending medications.
Remove Oversight Access in Therap (If Applicable)
- After the individual has been fully discharged, remove Neighborhood LTC Pharmacy’s oversight access in Therap
- This step helps protect privacy and ensures proper system access control
- If unsure how to remove access, consult your Therap administrator
Important Reminders
- Advance notice helps ensure medications are included with the individual
- Confirm internal medication records and documentation reflect the discharge
- Oversight access in Therap should be removed once the transition is complete (if applicable)
📝 Note: If the individual may return to your care in the future, notify Neighborhood LTC Pharmacy so important records can be retained.
